We have an 8 bedded unit at the Hospice in a tranquil location set in Renfrewshire countryside, with accommodation for family members.

Referral Criteria and Procedure

Referral Criteria
Patients can be referred to the Hospice by their GP, Hospital Consultant, Specialist Palliative Care Nurse, Community Nurse, or by self-referral to certain services, with the agreement of the family doctor.

Procedure
Referral forms can be obtained by contacting the Medical Secretary, via telephone (01505 705635) or email to wilma.jamieson@svh.co.uk

The form should be posted or faxed, outlining the main problems with copies of relevant letters. Urgent referrals can be made by telephone with documents sent at a later date.Except for planned respite admission, the Hospice does not have a waiting list. All referrals are scored and prioritised using the Palliative Outcome Scale. This is why it is important that as much information can be provided at the time of referral to avoid delays in responding to a patient's needs.
